Root Cause Analysis: How it Saved our Customers Engine

At Marspeck we use Root Cause Analysis (RCA). This technique helps question why the diesel engine problem occurred in the first place. It seeks to identify the origin of a problem using a specific set of steps, and in conjunction with our experience and factory support, to find the primary cause of the problem so that we can:

1. Determine what happened.
2. Explain why it happened.
3. Figure out a solution to reduce the likelihood that it will happen again.

Case Study:
During an inspection of a cruise ships engines for overheating, by applying RCA procedures, we found pitting to the underside of a cylinder head from cavitation. The root cause being eventually traced to incorrect gasket fitment due to foreign object entrapment (see the picture: can you guess what it was?).

Our involvement saved the customer serious damages and environmental disaster from loss of propulsion during confined water maneuvers. 

We can apply an RCA to almost any situation. Determining how far to go with that investigation requires good judgment and common sense.
